You confirm all details are correct and accurate and acknowledge that incorrect details may result in not receiving details in regards to your subscription or an inability to validate your identity for use of your subscriptions.
Possessing a Advantage Program Card or QR code does not determine whether your subscriptions are active or inactive. Losing your Advantage Program Card or QR code will not affect your subscription status. You can still access services without it, but we may require additional information to verify your identity.
ID photos that contain more than one person, are blurry, are obstructed, or do not show your full face will be rejected. You acknowledge that if the image is not recognizable as you, we may not be able to verify you are the Advantage Program holder and may deny services.
You can update your ID photo after approval, but it must be approved by our team prior to the new image being displayed.
